Participant Acknowledgment and Assumption of Risk
By registering for lessons, clinics, or events with Haag Pickleball Coaching, I acknowledge that participation in pickleball and related activities involves inherent risks, including but not limited to falls, collisions, overexertion, equipment-related injuries, and other risks that may result in serious injury.

I voluntarily assume all risks associated with participation and agree to participate at my own risk. I certify that I am physically capable of participating in pickleball activities and understand that I am responsible for consulting a physician regarding any medical concerns.

To the fullest extent permitted by law, I release and hold harmless Haag Pickleball Coaching, Coach Matt Haag, assistant coaches, volunteers, facility owners, and affiliated partners from any claims, demands, damages, or causes of action arising from my participation in lessons, clinics, or events, except where prohibited by law.

If registering a minor, I certify that I am the parent or legal guardian and consent to the minor's participation under these terms.
